- Ryu (リュウ Ryū)
- Son of Raoh, and the future inheritor of Hokuto Shin Ken after Kenshiro. At a young age, he already shows his wit and bravery, he also possess his fathers eye. Kenshiro was his personal mentor for some time (during the "Kōketsu", "Sava" and "Blanca" arc) before he left Ryu to be taken care of by Balga. In New Fist of the North Star, Kenshiro is waiting Ryu to grow up so he can battle against Ryu for the Hokuto Shinken successor title.

- Reina: A character created in 2006, Reina at first seemed to be the answer to this long-held question. She was a childhood friend of Raoh's that loved him.
- Eva: A character from Rei Gaiden: Sōkoku no Garō. She says outright that she had slept with Raoh.
- Sakuya: A character created in 2008 for the Ten no Haoh anime, Sakuya also loved Raoh.
- An unseen concubine: Probable, but the least popular theory.
