Other than a different title screen (which features a new artwork depicting Kenshiro and Raoh similar to the cover art of the [[Hokuto no Ken (Famicom)|first Famicom game]]), the two versions are otherwise identical.

==Story==

In 199X, the world was engulfed in nuclear flames. The seas evaporated. The ground was split. It seemed as if every creature on Earth was extinct. However, humanity refused to die... A lawless world where might makes right was born.

However, a man suddenly came out from the wilderness. Only death awaits those who bring forth the wrath of his eyes... The violent battles of the Legendary Savior of Century's End begins now!

*[[Kenshirō|Kenshiro]] - The successor of [[Hokuto Shin Ken]], an assassination art passed down from a single line of successors that carries with it a 2,000 year-old history. An invulnerable man who possesses an absolute power, he has studied several secret techniques, there are very few people in the world who could possibly defeat him. He is praised by the meek as the "Savior of Century's End" and feared by evildoers as the Angel of Death himself. The seven scars on his chest forms the shape of the Big Dipper.

*[[Heart]] - A giant man whose fatness consists of a thick rubber-like substance. Anyone or anything that strikes his large body will be sent flying away.

*[[Shin]] - One of the successors of [[Nanto Sei Ken]], a style which, in complete contrast to Hokuto Shin Ken, causes destruction from outside. He can launch vacuum waves from his sharp handstabs, which is said to be capable of cutting adversaries from faraway places.

*[[Jagi]] - The third of Kenshiro's honorary brothers. A violent individual who never truly mastered the ways of the fist, he harbors a hatred towards his younger Kenshiro for being chosen over him to succeed in the ways of Hokuto Shin Ken.

*[[Uighur]] - The warden of Cassandra, the City of Wailing Demons, and an underling of Raoh who reports directly to him. Not only does his gigantic figure towards over Heart, his agility is quick too. Although he specializes in the ways of the whip, his special technique, the "Tao of the Mongolian Dominator", which has been passed down since ancient Mongolia, carriers with it a dreadful power as well.

*[[Souther]] - The successor of [[Nanto Hōō Ken]] which stands at the top among the 108 schools of Nanto Sei Ken. He is also known as Souther the Holy Emperor, the Monarch of Nanto. He can shoot vacuum waves like Shin, but with a far greater power.

*[[Raoh]] - The eldest of Kenshiro's oldest, who is known as the King of Fist, the Conqueror of Century's End. He has the power to defeat his adversaries without laying a hand of them by using only his "tōki" fighting aura. A man who isn't afraid of even God himself, he harbors the ambition of pursuing a dominant order to the century's end era, unrestrained by the limits of Hokuto and Nanto.

*[[Falco]] - The successor of [[Gento Kō Ken]], a man also known as Falco the Gold, the young courageous general of the Imperial Capital. The art of Gento specializes in the annihilation of an opponent's body with the use of one's own fighting aura and it is said that does who thoroughly follow its path will have a permanent aura glowing on their hands.

*[[Han]] - The third Rashō (Asuran General) at the [[Land of Asura]]. A successor of [[Hokuto Ryū Ken]]. A man who believes than an unambitious person is not worth keeping alive and enjoys thwarting various kinds of attempts at taking his life. His unmatched invincible fists are said to be so fast that they don't even leave a shadow and because of that, they are known as "hurricanes".

*[[Hyoh]] - The second Rashō at the [[Land of Asura]]. A successor of [[Hokuto Ryū Ken]]. Although he is Kenshiro's true elder brother, his memory was sealed by Kaioh and he now opposes him as a master of Hokuto Ryū Ken.

*[[Kaioh]] - The first Rashō at the [[Land of Asura]]. A successor of [[Hokuto Ryū Ken]]. Raoh's true older brother, he possesses a power demonic fighting aura known as "matōki" from his thorough mastery of Hokuto Ryū Ken and now he seeks to take Kenshiro's life. He carries the title of Kaioh the Demon, the Devil King of Century's King.
