The game is based on the second anime series, ''Hokuto no Ken 2'', and covers primarily the first fifteen episodes of the series, where Kenshiro faces off against the ''Gento Kō Ken'' fighters. Because the series was still airing when the game was initially made, the game only goes up to the beginning of the "Land of Asura" arc. The final boss is the Nameless Asura, who was a minor villain in the series.
The North American version is simply titled ''Fist of the North Star'' and is not to be confused with the original ''Hokuto no Ken'' for the Famicom, which was unreleased overseas. Despite being based on ''Hokuto no Ken 2'', the North American cover artwork depicts Kenshiro fighting against Toki, who appears only in the first anime series.

* The game is titled ''Fist of the North Star'' instead of ''Hokuto no Ken 2'' (no other ''Fist of the North Star'' video game were released overseas for the NES).
* Background of the title screen was changed from black to blue. The color of Kenshiro's sprite was also changed the same way as well.
* The player's "aura" gauge is now a "bombs" gauge.
* The "abeshi" and "hidebu" power-ups were changed into generic white and black stars.
* Certain bosses, namely [[Barona]] and [[Buzori]], were made easier to defeat in the American version.
* Kenshiro's allies that appeared at the end of certain stages in the Japanese version were removed.
* The finishing moves for both, the player and the enemies, were removed as well.
The game was advertised in Viz Communications' short-lived eight-issue adaptation of the ''Fist of the North Star'' manga published in 1989. Although [[Toki]] appears on the American version's cover artwork sparring with Kenshiro (taken from a cel still used for episode 70 of the anime series), he does not actually appear in the game.
